Where is the online MELSA Reference Crossover Survey form?

The web form doesn’t load.

  • Check to see if other websites are available.  If no, contact your library’s technology support.  If yes, contact Chris at MELSA.
  • Run copies of the paper survey form provided in this manual and use them until access is restored.

The survey form doesn’t list my library first.

  • Click “Change Your Location” on the left-hand navigation bar.
  • Click on your library system in the drop down menu.
  • Wait for the page to reload.
  • Enter your branch library.
  • Your library system should be listed first.  If not, send email to Chris.
  • Continue to use online form until problem is corrected.

How can I be sure that the data I’m entering is being recorded?

  • Click “View Reports” on the left-hand navigation bar.
  • Leave the default “Detail” selected under “Select Report” on the right.
  • Leave the default “Today” selected under “Select Date Details” on the right.
  • Under “Select Location Details” click your library system under “County.”  Select your branch under “Library.”  Click “Run Report.”
  • Your data should be listed.  If not, send an email to Chris.
